Rybelsus (oral semaglutide): what it is, how to take it and what to eat

If you have been prescribed Rybelsus, the morning routine can raise questions: when to have breakfast, what to do about other tablets and how to eat if you feel nauseous.

Rybelsus is oral semaglutide authorised for adults with type 2 diabetes. It is taken once a day on an empty stomach with a little water. You must then wait at least 30 minutes before eating, drinking or taking other medicines [1]. It contains the same active ingredient as Ozempic, but the doses and routes of administration are different. Your doctor decides the dosing schedule.

What Rybelsus is and how it works

Rybelsus contains semaglutide, a medicine in the GLP-1 receptor agonist family. It mimics a signal from the gut that helps the pancreas release insulin when blood glucose rises. It also slows stomach emptying and may reduce appetite [1].

In Spain, it is available as a once-daily tablet. Ozempic contains the same molecule, but is given as a weekly injection [2]. If you searched for “Ozempic tablets”, you probably mean Rybelsus. Our guide to oral GLP-1 medicines explains the differences between oral formulations.

The fact that both contain semaglutide does not make their milligram doses equivalent. Only a small and variable proportion of an oral dose reaches the bloodstream [1]. Absorption also changes if the tablet is taken with food or too much water. The prescriber interprets this variation alongside your blood glucose readings and how well you tolerate the medicine.

That is why you cannot work out a home conversion from the injection pen. The product information warns that the effect of switching between routes is difficult to predict [1]. If you would prefer to avoid injections, discuss it at your medical appointment. Convenience, blood glucose control and whether the routine is manageable all matter when choosing a formulation.

Medicines in this family have different indications. You can read our guide to GLP-1 medicines and find out about the active ingredient in our semaglutide guide. Neither page replaces an assessment by someone who knows about your diabetes and other treatments.

Who Rybelsus is authorised for in Spain

Rybelsus is authorised for adults with inadequately controlled type 2 diabetes, alongside diet and exercise [1]. It can be used alone when metformin is not suitable, or in combination with other diabetes medicines. This indication does not mean that everyone with type 2 diabetes needs it.

Situation What the product information says What to review
Adults with type 2 diabetes Authorised indication [1] Blood glucose control, other treatments and tolerability.
Weight management without type 2 diabetes Outside the Rybelsus indication [1] Discuss options with your doctor that have this indication.
Under 18 years of age Safety and efficacy have not been established [1] Care from a specialist team.

Oral Wegovy also contains semaglutide, but it is a different medicine with its own indication for weight management in adults [3]. It is authorised in the European Union. This alone does not show that it is marketed in Spain. Nor does it make Rybelsus, Wegovy and Ozempic interchangeable.

At appointments, we hear a reasonable question: “If it is the same molecule, does it have the same effect?” The product information sets out different dosing schedules, and absorption by mouth is variable [1][2]. The answer for an individual depends on their blood glucose control, medical history and follow-up. We do not recommend one route over another.

If you take insulin or a sulphonylurea, your medical team should assess the risk of low blood glucose [1]. Missing a meal can complicate this. Tell us your schedule and medicines so we can adapt meals without changing your prescription.

How do you take Rybelsus?

Taking it on an empty stomach is part of using the medicine safely and effectively. The product information recommends taking it after fasting for at least 8 hours. Swallow it whole with a sip of water, up to 120 ml. Wait at least 30 minutes before eating breakfast, drinking anything else or taking other medicines by mouth [1].

Preparing breakfast the night before makes it easier to wait. It may also help to keep your pill organiser separate from your first meal. If you take several medicines in the morning, take the list to your pharmacist. The product information reports lower semaglutide absorption when several tablets are taken at the same time [1]. Do not change the timing of a medicine with specific instructions on your own.

The new formulation uses round tablets of 1.5 mg, 4 mg and 9 mg. The old formulation used oval tablets of 3 mg, 7 mg and 14 mg. The equivalent pairs are 1.5/3, 4/7 and 9/14 mg [1][4]. The new formulation delivers more semaglutide into the body, which is why it contains fewer milligrams for an equivalent effect. Check the name, shape and dose on the pack with your pharmacy.

The current product information also lists 25 mg and 50 mg strengths, which are not marketed in Spain as at 26/09/2026; the 1.5 mg, 4 mg and 9 mg strengths are dispensed here. The usual starting dose for the new formulation is 1.5 mg daily for one month, followed by 4 mg. Any later increase requires at least another month on the current dose and medical assessment [1]. These details describe the product information; they do not tell you which dose is right for you.

Rybelsus is taken as one tablet a day. Do not combine tablets to make a higher dose [1]. If you miss a dose, skip it and take the next one the following day [1]. Do not take a double dose. If you mix up packs of the two formulations, check with a healthcare professional before taking another tablet [4].

Switching from Ozempic requires a prescription and follow-up. The product information warns that exposure after changing route is difficult to predict [1]. If you also use insulin or sulphonylureas, your prescriber may review those doses to limit hypoglycaemia (low blood glucose) [1]. We do not adjust medicines.

Rybelsus increases exposure to levothyroxine, a thyroid hormone, by around 33%; the product information says thyroid parameters should be considered for monitoring if you take it [1]. If you take warfarin or acenocoumarol (Sintrom), the semaglutide product information recommends more frequent INR monitoring when starting treatment [1]. Always discuss this with your prescriber.

Rybelsus price and reimbursement in Spain

The price of Rybelsus depends on the formulation and whether it is reimbursed by Spain’s National Health System. With a private prescription, the retail price starts at €123.53. With a National Health System prescription and, where required, prior authorisation, the medicine has a reduced patient contribution; what you pay depends on your circumstances—some people are exempt—and you will see the amount when the prescription is dispensed.

The National Health System reimburses certain formulations with prior authorisation for type 2 diabetes. The criteria include a body mass index of 30 kg/m² or higher and combination therapy with other antidiabetic medicines when adequate control has not been achieved [5]. The authorised use as monotherapy is listed as not reimbursed [5]. Your doctor and pharmacy can check whether your circumstances meet the criteria.

Authorised indication and reimbursement answer different questions. Having a valid prescription does not guarantee that the National Health System will cover the pack. If your treatment changes, the applicable conditions may change too. Before relying on an amount, confirm the formulation, authorisation and patient contribution with your pharmacy.

Compare prices and coverage in our guide to the cost of GLP-1 treatments. For information about patents and possible generics, see our semaglutide guide. None of these factors is a reason to change formulation without medical advice.

What side effects can Rybelsus cause?

Digestive side effects are the most common. In the initial trials, with doses up to 14 mg of the old formulation, nausea occurred in 15%, diarrhoea in 10% and vomiting in 7% of people taking oral semaglutide [1]. Nausea and vomiting were more common with the higher 25 mg and 50 mg doses [1].

Effect Frequency in the product information How food may help
Nausea Very common; 15% in the trials [1] Small, slow, lower-fat meals; stop when you feel comfortably full.
Diarrhoea Very common; 10% in the trials [1] Replace fluids in small sips and choose foods you can tolerate.
Vomiting Common; 7% in the trials [1] Resume fluids gradually when you can; seek advice if you cannot keep them down.
Constipation Common, from 1 in 100 to fewer than 1 in 10 people [1] Enough water, gradually increased fibre and movement as tolerated.
Reduced appetite Common, from 1 in 100 to fewer than 1 in 10 people [1] Plan meals that provide protein, vegetables and energy.

These measures may improve tolerance; they do not replace a review of persistent or severe side effects. If you eat much less for several days, the issue is no longer just which foods suit you. Hydration, blood glucose and treatment should be reviewed with the prescribing professional [1].

A large meal can be difficult when you feel nauseous. Try a smaller portion and save the rest for later. If you have diarrhoea, focus on being able to drink; if you are constipated, increase fibre gradually. Avoiding many food groups to prevent symptoms can leave you short of nutrients [6].

At appointments, we also look at when the discomfort occurs. If it starts after a dose increase, note the date and tell your doctor. Food can ease symptoms, but the decision to continue or change the medicine is theirs.

What to eat while taking Rybelsus

When your appetite is lower, a small meal still needs to provide protein and variety. At appointments, we see people who get to the afternoon having had only coffee and some bread because they were hardly hungry. First, we review what they have managed to eat, their medicine timings and any symptoms. Then we adapt the plan to their routine.

One possible breakfast, after waiting for the tablet, is plain yoghurt with fruit and oats, or toast with egg and tomato. For lunch, a dish with pulses, vegetables and rice is easy to prepare in advance. For dinner, you could have fish with potato and cooked vegetables. These are general examples; diabetes and other treatments may mean changes are needed.

A joint advisory from scientific societies on GLP-1 treatment recommends assessing dietary quality, meeting nutrient needs, paying attention to protein and maintaining strength exercise [6]. We apply this without imposing one target on everyone. Needs vary with age, activity, kidney function and the rest of a person’s medical history.

When your appetite is lower, every meal still needs to look after your strength and health.

What we see in appointments

If nausea makes it hard to finish a meal, divide the food you can tolerate into smaller portions. Do not force yourself to eat a large portion to meet a timetable. Nor should lack of appetite become deliberate restriction. Look for a plan you can maintain and tell the person supporting you if you are eating much less.

Hydration needs attention if you develop vomiting or diarrhoea. Once you have waited the required time after Rybelsus, sip fluids regularly throughout the day. If you cannot keep fluids down, follow the advice in the warning signs section. The product information warns of dehydration and associated kidney problems caused by digestive side effects [1].

For meal ideas so you do not have to repeat the same solution every day, see what to eat with GLP-1 treatment. If you are concerned about losing strength, our guide to protein and muscle during treatment explains how to protect them. Examples are tailored at appointments.

If your doctor decides to stop it

Stopping Rybelsus should be planned with the person managing your diabetes. Your blood glucose and other medicines may need to be reviewed. We do not have a specific figure for weight regain after stopping Rybelsus that can predict what will happen in your case. Results from studies of other formulations or indications cannot automatically be applied here.

While you are taking the medicine, we can build meals that will continue to work afterwards. This includes realistic mealtimes, protein sources you enjoy, practical shopping and strength-based activity adapted to you. The aim is to preserve your ability to eat well and keep moving, even if your appetite changes.

If you feel hungrier when the treatment is reduced or stopped, mention it at your follow-up. We will review satiety, blood glucose, sleep and the structure of your meals. Avoiding a very restrictive response often makes a sustainable eating plan easier. Your medical plan determines what happens with the medicine; your nutrition plan is adjusted to fit it.

Our guide to weight changes after stopping semaglutide explains the available evidence for other formulations. Bear this limitation in mind when reading it.

When it is not indicated or extra caution is needed

The formal contraindication in the European product information is an allergy to semaglutide or any of the tablet’s ingredients [1]. Other situations are not formal contraindications, but may mean it should not be used or that extra caution is needed.

  • Rybelsus must not be used during pregnancy or while breastfeeding [1].
  • If you are planning a pregnancy, the product information says to stop it at least two months beforehand [1].
  • Oral semaglutide is not recommended in severe gastroparesis [1].
  • If you have had pancreatitis or have diabetic retinopathy, you need a specific assessment and follow-up [1].
  • If you have or have had an eating disorder, mention it before starting treatment [6].

Your doctor will assess whether you also need mental health support. Advanced kidney or liver disease, other medicines and difficulty following the fasting instructions should be reviewed [1]. The product information does not establish safety and efficacy in people under the age of 18 [1]. The decision always depends on an individual medical assessment.

If you live outside Spain

Authorisation, reimbursement and available formulations of Rybelsus can vary by country. A European product information leaflet does not confirm which pack is dispensed in Mexico, Argentina or Colombia. Ask your medical team and local pharmacy to check the current registration, the dose on the pack and applicable coverage.

The dosing instructions described here come from the Spanish product information [1]. If you have a formulation from another market, follow the leaflet in your pack and ask about any differences. The new and old formulations may contain different amounts of semaglutide for an equivalent effect [4]. Do not calculate this equivalence yourself.

If you live outside Spain and already have a prescription, our online nutrition support can help you organise meals and manage symptoms. Medication reviews are the responsibility of your local prescriber.

Frequently asked questions about Rybelsus

Is there an Ozempic tablet?

Ozempic is an injection. Rybelsus is oral semaglutide for type 2 diabetes, with its own dosing schedule and formulations [1][2]. See oral GLP-1 options.

Is Rybelsus as effective as Ozempic?

Both contain semaglutide, but Rybelsus absorption is lower and more variable [1]. Their doses cannot be compared milligram for milligram. Your doctor will assess which route and schedule suit your blood glucose control.

Is Rybelsus oral Wegovy?

No. Oral Wegovy is a different medicine containing semaglutide, with its own European indication for weight management in adults [3]. Rybelsus is authorised for type 2 diabetes [1].

Can I take it with coffee or breakfast?

No. Swallow it whole with up to 120 ml of water and wait at least 30 minutes before coffee, food or other oral medicines [1].

What should I do if I miss a dose?

Skip the missed dose and take the next one the following day. Do not take two tablets to make up for it [1].

Can I take two tablets of the old dose to match the new formulation?

No. The formulations are absorbed differently and the product information says to take one tablet a day [1][4]. Check the pack and prescription with your pharmacy.

When to see a doctor

When to seek medical help

Our role. With GLP-1 treatment, we support your eating during and after treatment: getting enough protein, preserving muscle, doing strength training and building habits that support your weight when you stop. We do not prescribe the medicine, adjust the dose or replace your prescriber. If the medicine is causing problems, speak to your doctor.

Arrange a medical appointment without waiting to change your diet if you notice any of these signs:

  • Severe, persistent pain in the upper abdomen that spreads to your back, with vomiting [1].
  • Vomiting or diarrhoea that prevents you from drinking for more than a day [1]
  • Pain in the upper right abdomen, yellow skin or eyes, or very pale stools [2].
  • You have diabetes, take insulin or tablets that lower blood glucose, and are having hypos [1].
  • Your vision has changed and you have diabetic retinopathy [1]
  • You are losing weight very quickly, with marked weakness and loss of strength [6]

What to do. If you have pain spreading to your back with vomiting, go to A&E today or call 112: this could be pancreatitis (inflammation of the pancreas). If you have pain in the upper right abdomen, yellow skin or eyes, or very pale stools, also go to A&E today. If you cannot keep fluids down, contact your doctor today; if you also feel dizzy when standing or are barely passing urine, go to A&E. If a hypo is severe (confusion, inability to swallow or needing help from someone else), call 112; for mild hypos or a change in vision, contact your prescriber today or tomorrow; if you suddenly lose vision, go to A&E. If you are losing weight very quickly and feel weak, contact your doctor this week: strength and protein are addressed, and the treatment plan is reviewed.

Most troublesome GLP-1 side effects are digestive, temporary and manageable. These are the few signs that mean you should stop and seek advice.
