What is orforglipron, and how does it differ from Rybelsus?
Orforglipron is a tablet that acts as a GLP-1 receptor agonist. It mimics GLP-1, a gut hormone that reduces appetite and helps regulate blood glucose. In countries where it has been authorised, it is marketed as Foundayo [1][2]. Unlike semaglutide, which is a peptide, it is a small molecule.
The practical difference from oral semaglutide (Rybelsus) is how you take it. Foundayo is taken once a day, at any time, with or without food [4]. Rybelsus is taken on an empty stomach with a small amount of water, and you have to wait before eating, drinking or taking other medicines [5]. Find out more in how GLP-1 tablets work. The dosage for each medicine is set by its product information and the person prescribing it.
Where does its authorisation stand?
| Region | Status as at 26/09/2026 |
|---|---|
| United States | The FDA authorised Foundayo in April 2026 for weight loss and weight maintenance in adults with obesity, or with overweight and at least one weight-related condition [1]. |
| United Kingdom | The MHRA authorised it in August 2026 for weight management and type 2 diabetes in adults. It is not yet funded by the UK public health service [2]. |
| European Union | The EMA is assessing the application for obesity and type 2 diabetes. In September 2026, it was still at the outstanding-questions stage [3]. |
| Spain | Not authorised; no marketing date. The European decision has to come first [3]. |
Authorisation in another country does not mean it is available in Spanish pharmacies. The European assessment could result in approval, a different indication or refusal. Decisions on price and reimbursement in Spain would come afterwards. That is why we cannot give an arrival date.
If you read that it is “already approved”, check which country. The United Kingdom is not part of the European Union [2][3]. There is currently no price in Spain or known access arrangements. Our GLP-1 medicines guide summarises the options available here.
What do the trials show?
The main obesity trial followed around 3,100 adults without diabetes for 72 weeks, alongside dietary and physical activity advice [6]. At the highest dose, average weight loss was 11% of body weight, compared with 2% with placebo [6]. At the lower doses studied, it was around 7–8% [6].
This comparison is with placebo and does not tell us whether it is preferable to another GLP-1 medicine. The average does not predict what will happen for any one person either. The indication, tolerability, duration of treatment and starting health all play a part. That is why we care about how much of the weight lost is muscle and which habits help maintain the change.
A tablet being easier to take does not change who should assess whether treatment is right for you.
What we see in practice
When a new medicine comes along, two questions often come up in consultations. The first is the assumption that a tablet does not need medical assessment. The second is putting lifestyle changes off until it is available. You can start those changes today with adequate meals, activity adapted to you and medical follow-up when needed.
We also hear comparisons between different trials. They can be confusing because the people studied and the methods are not the same. If you are already taking another GLP-1 medicine, do not switch or stop it because of a news story. Take your questions to the person who prescribes it and review your diet with your dietitian.
What can you do while you wait for it to reach Spain?
You do not need to wait for orforglipron to look after your diet. If you already take a GLP-1 medicine prescribed by your doctor, the first priority is making sure reduced appetite does not leave you with meals that are too small. A simple plate includes a source of protein, vegetables, carbohydrates and olive oil. The amounts are tailored to you, your preferences and what you tolerate.
To plan your week, see our sample weekly menu while taking a GLP-1. For protein and strength, read our guide to maintaining muscle during treatment. Protein needs are worked out for each person, and strength training is adapted to your starting point and your healthcare team’s advice.
If you are not taking one yet, the decision to start is for your doctor. If you have had an eating disorder, discuss it beforehand with your doctor and a mental health professional. If you start treatment, our GLP-1 nutrition support covers protein, strength, digestive symptoms and a plan for when you stop.
Your first step this week
Review a typical meal and note whether it includes a source of protein you tolerate well. This gives you a starting point for adjusting your plan in a consultation.

Frequently asked questions about orforglipron
Can you get orforglipron in Spain?
No. It is not authorised in the European Union [3], and approval in the United States or the United Kingdom does not make it available for sale here [1][2]. Do not buy it through unregulated channels. The risks are explained in buying GLP-1 medicines without a prescription.
When will it reach Spain?
There is no confirmed date. The European application is under assessment [3]. Any date circulating now is an estimate and depends on the European decision and subsequent marketing.
Is it the same as Rybelsus?
No. Both are GLP-1 tablets, but they contain different active substances. According to its US product information, Foundayo can be taken with or without food [4]. Rybelsus is taken on an empty stomach, with a wait before eating or drinking [5]. Do not change medicines without speaking to the person who prescribes yours.
When to see a doctor
When to seek medical advice
If you already take a prescribed GLP-1 medicine, these signs can help you recognise when to seek medical attention.
When you take a GLP-1 medicine, we support your diet during and after treatment: eating enough protein, preserving muscle, doing strength training and building habits that can help maintain your weight when you stop. We do not prescribe the medicine, adjust the dose or replace the person who prescribes it. If the medicine is causing problems, speak to your doctor.
Seek medical advice without waiting to change your diet if you notice any of these signs:
- Severe, persistent pain in the upper abdomen that spreads to your back, with vomiting [7].
- Vomiting or diarrhoea that prevents you from drinking for more than a day [7].
- Pain in the upper-right abdomen, yellow skin or eyes, or very pale stools [7].
- You have diabetes, take insulin or tablets that lower blood sugar, and experience hypoglycaemia [8].
- Your vision has changed and you have diabetic retinopathy [7].
- You are losing weight very quickly, with marked weakness and loss of strength [9].
If you have pain that spreads to your back with vomiting, go to A&E today or call 112: it could be pancreatitis (inflammation of the pancreas). If you have pain in the upper-right abdomen, yellow skin or eyes, or very pale stools, go to A&E today too. If you cannot keep fluids down, contact your doctor today; if you also feel dizzy when standing or are barely passing urine, go to A&E. If low blood sugar is severe (confusion, inability to swallow or needing help from another person), call 112; for mild episodes or a change in vision, contact the prescriber today or tomorrow; if you suddenly lose vision, go to A&E. If you are losing weight very quickly and feel weak, contact your doctor this week: your strength and protein intake can be addressed, and your treatment reviewed.
Most troublesome effects of GLP-1 medicines are digestive, temporary and manageable. These are the few signs that mean you should stop and seek advice.
